It’s similar to a prescription but functions differently. A physician or nurse practitioner completes this form to authorize you to purchase cannabis from a licensed seller. It specifies your daily gram limit and is valid for up to one year.

Once you have an account, open the medical document intake form through our partner clinic. Have your provincial health card ready — you’ll need it to verify your identity.

Complete the required fields with your health card and medical history. All information is encrypted, confidential, and shared only with your consulting practitioner.

At the end of the form you’ll see a calendar to schedule a brief video appointment. A licensed physician or nurse practitioner will review your information and confirm your authorization.

If you have an existing document from another licensed producer, you can transfer it to PLC. Email a copy to our secure intake team and we’ll register it to your account.

- How long does the process take?
- The intake form takes about 5 minutes. Consultations are typically booked within a few days, and the appointment itself is about 5–10 minutes. Most patients have their document within a week.
- Is there really no cost?
- Correct. The physician consultation and document are provided at no cost to you. There is no subscription and no obligation to purchase from PLC afterwards.
- What conditions qualify?
- Medical cannabis can be authorized for a wide range of conditions including chronic pain, anxiety, insomnia, PTSD, arthritis, and more. Your consulting practitioner will determine if cannabis is appropriate for your situation.
- How long is the document valid?
- Medical cannabis documents are typically valid for up to 12 months. When it's time to renew, the process is the same — a brief consultation and updated authorization.
- Can I use my document with other licensed producers?
- Your document is registered with PLC as your licensed producer. If you'd like to split your prescription across multiple LPs, speak with your practitioner during the consultation.
